Is your young person currently moving from children’s to adult social care, or have they recently made this transition?
The Council’s Adult Social Care Service is developing a new policy for young people with disabilities moving into adulthood. Whilst it will not solve every problem, they hope that the new policy will improve things for those young people and their families.
They want to hear about the issues that families have experienced in recent times – what worked well and what worked less well. Please come along to this meeting and talk about your experiences. Booking essential.
